If you enroll within your 31-day enrollment period, you may elect Supplemental Life Insurance for yourself up to 10 times your Benefits Eligible Earnings or $1,000,000, whichever is less, and Spouse or Domestic Partner Life Insurance up to $30,000 without providing Evidence of Insurability. You may submit your prior-year Form W-2 to HR Services within 31 days of your date of hire to receive the maximum level of coverage based on your earnings. Contact HR Services for instructions.
If you do not enroll within your 31-day enrollment period, generally you will not receive Supplemental Life, Spouse or Domestic Partner Life, Child Life or Supplemental AD&D Insurance for your year of hire. If you elect (or increase) Employee Supplemental Life, Spouse or Domestic Partner Life Insurance coverage at a later date, you will be required to provide Evidence of Insurability.
