Transition of Care

Transition of care provisions are designed to prevent any interruption of coverage if you change coverage options and are currently undergoing treatment for a serious illness or health issue (e.g., chemotherapy). If your Medical Plan coverage will change in the next year and your health care provider providing ongoing services is not in your new coverage’s network, you may be eligible for transition of care. Contact your Medical Plan  administrator for more information.
