If you participate in a UHC medical plan option and which to undergo fertility treatments, you must enroll in the Fertility Solutions program to receive coverage. Upon enrollment in the program, you will have access to care from a specialized infertility Center of Excellence. The program is designed to help you maximize your fertility benefit and select effective treatment options. You may contact the Fertility Solutions team at 1-866-774-4626 to identify a network facility from which you may receive treatment or for assistance negotiating payment with an out-of-network provider. Certain exclusions for infertility treatments may apply.
